Error: "Fail to copy file to DM File Store because the file...does not exist" when emailing from Document Manager

In MYOB Document Manager (DM) the following error "Fail to copy file to DM File Store because the file C:\Users\<username>\AppData\Roaming\DocumentManager\Temp\<filename>.msg does not exist" appears when sending trying to send emails.
This occurs because the DM Toolbar is missing or disabled in Microsoft Outlook.
